The tenant will obtain & provide proof of tenant liability insurance before keys are issued. All costs & risks incurred by failure to provide proof of insurance will be born by the tenant. Failure to provide proof or cancellation of insurance will end the lease, the tenant will vacate immediately, and the tenant will remain responsible for rent until the premises are relet. Smoking is prohibited within any building or within 4m of any door or window. The property will be subject to inspection at approximately six month interval. The tenant acknowleges that the lease is for the occupancy described above, that no persons not named in this application or the lease will be resident in the premises.
I hereby authorize Dockrey Apartments or their agent(s) to obtain at any time consumer credit report about me, to contact previous landlords to obtain information about my previous tenancies, to contact agencies that provide landlord information, to contact my references, and to take any other reasonable steps necessary to assess this Rental Application, or for any renewal or associated tenancy issue. I further acknowledge that Dockrey apartments will disclose, use and collect my personal information in accordance with Dockrey Apartments

I represent that the information provided in this application is true and correct. I hereby offer to lease from Dockrey Apartments the suite referred to above as the terms stated and if accepted, undertake to execute a lease in the standard form used by the landlord for the property.
